Quick answer

A liver doctor is a hepatologist — a specialist in liver disease, usually a gastroenterologist with extra liver training. For common problems such as grade 1–2 fatty liver, mildly raised SGPT or metabolic risk, an experienced physician or diabetologist can manage you well. See a gastroenterologist or hepatologist for jaundice, hepatitis B or C, cirrhosis signs, very high enzymes, or when scarring is suspected.

01Types of doctors who treat liver problems

  • Hepatologist: super-specialist in liver diseases — hepatitis, cirrhosis, liver failure, transplant evaluation.
  • Gastroenterologist: treats the digestive system including the liver; performs endoscopy and manages most liver diseases.
  • Physician (MD Medicine) or diabetologist: manages metabolic fatty liver, diabetes, cholesterol and weight — the main drivers of fatty liver.
  • Infectious disease specialist: for viral hepatitis with HIV or other infections.
  • Liver transplant surgeon: for end-stage liver disease.

02Which doctor for fatty liver?

Fatty liver is a metabolic condition. For grade 1 or 2 fatty liver with normal or mildly raised liver enzymes, a physician or diabetologist who can address weight, sugar, triglycerides and blood pressure is the most practical first doctor. They will calculate a fibrosis risk score, order a FibroScan if needed and refer to a hepatologist if there is significant scarring. Our guide to fatty liver grades explains what your ultrasound means.

03When you need a gastroenterologist or hepatologist

  • Jaundice (yellow eyes or skin) or dark urine.
  • Hepatitis B or C positive test.
  • ALT/AST several times above normal, or persistently raised without explanation.
  • FibroScan showing significant fibrosis or cirrhosis.
  • Swelling of the abdomen or legs, vomiting blood, black stools, confusion.
  • Liver lumps or lesions on ultrasound.

04Common liver tests and what they mean

  • ALT (SGPT) and AST (SGOT): enzymes released when liver cells are irritated.
  • ALP and GGT: raised in bile duct problems, alcohol use and some medicines.
  • Bilirubin: high in jaundice — see what high bilirubin means.
  • Albumin and INR: show how well the liver is making proteins.
  • Platelets: fall in advanced liver disease.
  • Viral markers: HBsAg, anti-HCV.
  • Imaging: ultrasound, FibroScan, sometimes CT or MRI.

05What happens at a liver consultation

  1. History: alcohol, medicines and supplements, weight changes, diabetes, family history, symptoms.
  2. Examination: weight, waist, BP, signs of liver disease.
  3. Review of existing reports and new tests where needed.
  4. Diagnosis of the type and stage of liver disease.
  5. A plan: lifestyle, medicines, vaccinations (hepatitis A and B), follow-up schedule.

06How to prepare for a liver appointment

Bring all previous liver reports in date order — ultrasound, liver function tests, viral markers, FibroScan — because trends matter more than single values. Write down your daily alcohol intake honestly, every medicine and supplement you take (including Ayurvedic, homeopathic and gym products), your weight history and any family history of liver disease or diabetes. If you have symptoms, note when they started. Fasting is not usually required for a consultation, but some blood tests and FibroScan need a few hours of fasting, so ask when booking.

07Raised SGPT: common reasons and next steps

A mildly raised SGPT (ALT) is one of the most common reasons people search for a liver doctor. Frequent causes include fatty liver, alcohol, recent viral illness, intense exercise, certain medicines and supplements, and thyroid problems. Less common causes are hepatitis B or C, autoimmune hepatitis, iron overload and Wilson's disease. The usual approach is to repeat the test after a few weeks, check for hepatitis viruses, do an ultrasound, review medicines and address weight and alcohol. Persistently raised or very high values need specialist evaluation.

08Liver problems in people with diabetes

People with type 2 diabetes have a much higher chance of fatty liver and of it progressing to fibrosis. International guidelines recommend checking a fibrosis risk score in adults with diabetes even if liver enzymes are normal. A doctor who manages diabetes and liver health together can choose diabetes medicines that also benefit the liver and track both conditions in one plan. Read our diabetologist guide.

09Online liver consultation: what works

Many liver follow-ups suit online consultation: reviewing new reports, adjusting diet and medicines, and planning tests. Upload your reports on WhatsApp before the call. An in-person visit is needed for examination when there are new symptoms, or for FibroScan and other procedures. Online consultation saves travel time for patients outside Delhi NCR.

10Medicines and supplements that can harm the liver

Paracetamol overdose, some antibiotics, anti-TB drugs, certain painkillers, anabolic steroids, some herbal and "body-building" supplements, and unregulated detox products can injure the liver. Always tell your doctor everything you take, including Ayurvedic or herbal products, and never exceed recommended doses.

11Protecting your liver

  • Keep a healthy waist size and body weight.
  • Exercise at least 150 minutes a week.
  • Avoid or limit alcohol.
  • Get vaccinated against hepatitis A and B.
  • Avoid unnecessary medicines and supplements.
  • Control diabetes, cholesterol and blood pressure.

12Ayurveda and liver care

Ayurveda has a long tradition of liver-supporting diets and herbs. Used under a qualified doctor, alongside proper diagnosis and monitoring, it can complement care for metabolic fatty liver. Be cautious of products with undisclosed ingredients; some herbal mixtures have caused liver injury. The best results come from combining evidence-based testing with lifestyle-focused care.
