How many grades of fatty liver are there? What grade 1, 2 and 3 fatty liver mean on ultrasound, which tests matter more, and how to reverse it.

Quick answer

Ultrasound reports usually grade fatty liver from 1 (mild) to 3 (severe) based on how bright the liver looks. Grade 1 means a small amount of fat; grade 3 means a lot. But the grade does not show liver damage or scarring — that needs blood tests and a FibroScan. The good news: fatty liver, especially grades 1 and 2, can often be reversed with 7–10% weight loss, exercise, diet change and treating diabetes and cholesterol.

01What is fatty liver?

Fatty liver means fat makes up more than about 5% of the liver. The most common type today is metabolic dysfunction-associated steatotic liver disease (MASLD), earlier called NAFLD, linked to excess weight, belly fat, diabetes, high triglycerides and inactivity. Alcohol-related fatty liver is the other main type. In India, studies suggest fatty liver affects a large share of urban adults, many with normal body weight but high belly fat.

02How many grades of fatty liver are there?

Ultrasound reports commonly use three grades:

  • Grade 1 (mild): slightly increased brightness of the liver; the diaphragm and blood vessels are seen clearly.
  • Grade 2 (moderate): more brightness; the blood vessel walls are harder to see.
  • Grade 3 (severe): marked brightness; the back of the liver and diaphragm are poorly seen.

Grading is somewhat subjective and can vary between machines and radiologists. It tells you about the amount of fat, not about inflammation or scarring.

03What is grade 1 fatty liver?

Grade 1 is the mildest stage, often found incidentally on an ultrasound done for another reason. Most people have no symptoms. It is a warning sign rather than a disease to panic about: it signals that metabolism is under strain. With lifestyle changes, grade 1 fatty liver frequently disappears on follow-up ultrasound within months. Ignored, it can progress, especially if diabetes, weight gain or alcohol continue.

04Grade 2 and 3: when to take it more seriously

Higher grades usually mean more fat and often go with diabetes, high triglycerides or obesity. They raise the chance of inflammation (steatohepatitis, MASH), which can lead to scarring (fibrosis) and eventually cirrhosis. People with grade 2 or 3 fatty liver should have liver blood tests and a fibrosis assessment, and should treat related conditions actively.

05Tests that matter more than the grade

  • Liver function tests: ALT (SGPT), AST (SGOT), GGT — raised levels can suggest inflammation, though normal results do not rule it out.
  • FIB-4 score: a simple calculation from age, AST, ALT and platelets that estimates scarring risk.
  • FibroScan (transient elastography): measures liver stiffness (fibrosis) and fat (CAP score) painlessly.
  • Metabolic tests: HbA1c, fasting sugar, lipid profile, thyroid, and blood pressure.

06How to reverse fatty liver

  1. Lose weight gradually: 5% reduces liver fat; 7–10% can reduce inflammation and even early fibrosis.
  2. Exercise: at least 150 minutes a week of brisk activity plus two strength sessions; exercise helps even without weight loss.
  3. Diet: cut sugar, sweet drinks, fruit juices, refined flour and fried foods; choose vegetables, dal, whole grains, nuts and lean protein.
  4. Alcohol: avoid or minimise.
  5. Treat diabetes, cholesterol and BP.
  6. Sleep 7–8 hours and manage stress.

07A sample day of eating for fatty liver

  • Morning: water, a handful of soaked nuts; tea or coffee without sugar.
  • Breakfast: vegetable poha or upma with peanuts, besan chilla, or oats with curd.
  • Lunch: two multigrain rotis, dal, a large bowl of vegetables, salad and curd.
  • Evening: roasted chana, sprouts or a fruit (not juice).
  • Dinner: early and light — paneer, tofu, fish or chicken with vegetables; small portion of rice or roti.

Avoid sweets, sugary drinks, packaged snacks, bakery items and late-night eating. Small, consistent changes beat crash diets.

08Fatty liver and diabetes: a two-way link

Fatty liver and type 2 diabetes feed each other. Insulin resistance drives fat into the liver, and a fatty liver releases more sugar into the blood. People with diabetes have a higher risk of fatty liver progressing to fibrosis, so they should be screened with liver tests and a fibrosis score. Controlling diabetes improves the liver; some diabetes medicines also help reduce liver fat. A doctor who manages both conditions together gives the best results — see our diabetologist guide.

09Medicines and Ayurveda for fatty liver

There is no single pill that removes liver fat in everyone. Doctors may use specific medicines for patients with diabetes or proven inflammation, and some newer drugs are approved for certain stages in some countries. Ayurvedic approaches emphasise diet, routine and herbs; any herbal product should come from a qualified doctor, since some can harm the liver. Avoid self-medicating with "liver detox" products.

10Symptoms to watch for

Most fatty liver causes no symptoms. Seek care for yellow eyes or skin, swelling of the abdomen or legs, vomiting blood, black stool, confusion or easy bruising — these suggest advanced disease. Mild right-upper abdominal discomfort or tiredness can occur but are not specific.

11Follow-up schedule

For grade 1 with normal liver tests and low fibrosis risk, recheck after 6–12 months of lifestyle change. For grade 2–3 or abnormal tests, follow up more closely with a doctor who can track FibroScan results over time. Read which doctor to consult for fatty liver to choose the right specialist.

FAQFrequently asked questions

How many grades of fatty liver are there?

Ultrasound usually grades fatty liver as 1 (mild), 2 (moderate) and 3 (severe) based on liver brightness.

Is grade 1 fatty liver dangerous?

Usually not immediately, but it is a warning sign. With lifestyle changes it often reverses within months.

Can grade 2 fatty liver be cured?

Often yes. Losing 7–10% of body weight, exercise and controlling sugar and lipids can reduce or clear liver fat.

Does fatty liver grade show liver damage?

No. The grade shows fat. FibroScan and blood tests assess inflammation and scarring.

Which food is best for fatty liver?

Vegetables, dal, whole grains, nuts, fish and lean protein, with very little sugar, sweet drinks, refined flour and fried food.

Can thin people have fatty liver?

Yes. Lean fatty liver is common in India, often linked to belly fat, insulin resistance and diet.

How do I book a fatty liver consultation?

WhatsApp +91 70424 24269 or call +91 80109 31122. Consultation fee is ₹800.

How long does it take to reverse fatty liver?

Many people see improvement in liver fat and enzymes within three to six months of consistent weight loss and exercise.

Can fatty liver come back?

Yes, if weight, diet and sugar control slip. Maintaining healthy habits keeps the liver healthy long term.

Is FibroScan painful?

No. It feels like a gentle tap on the right side of the abdomen, takes about 10 minutes and needs a few hours of fasting beforehand.
