Quick answer

For concerns about breast development in girls and women — delayed growth, very small or uneven breasts — start with a gynaecologist, who may involve an endocrinologist if hormones are involved. Men with breast enlargement (gynaecomastia) should see a physician or endocrinologist, with a plastic or breast surgeon for persistent cases. Any new lump, nipple discharge or skin change needs prompt evaluation.

01Normal breast development

Breast development usually begins between 8 and 13 years of age and continues for several years. Size depends mainly on genetics and body fat. It is normal for one breast to be slightly larger, and for development to continue into the late teens. Breasts also change with the menstrual cycle, pregnancy, breastfeeding, weight change and menopause.

02When to see a doctor about breast growth

  • No breast development by age 13.
  • Development that starts before age 8.
  • Very marked asymmetry causing distress.
  • Breast changes with irregular periods, excess hair or galactorrhoea (milky discharge).
  • Rapid breast enlargement in men or boys.
  • Any lump, pain that persists, nipple discharge, or skin dimpling.

03Which doctor for which situation

  • Gynaecologist: breast development concerns in girls and women, with hormonal or menstrual issues.
  • Paediatric endocrinologist: early or delayed puberty in children.
  • Endocrinologist: hormone problems such as thyroid disorders, high prolactin or low oestrogen.
  • Breast surgeon: lumps, discharge, persistent pain or suspicious changes.
  • Plastic surgeon: cosmetic concerns, significant asymmetry or persistent gynaecomastia.

04Can breast size be increased without surgery?

Breast size is largely determined by genes and fat. Exercises strengthen the chest muscles under the breast and improve posture, which can change appearance slightly, but do not grow breast tissue. Creams, pills, oils and "breast growth" supplements have no reliable evidence; some contain hormone-like substances that may be unsafe. Hormonal treatment is used only when a real hormone deficiency is diagnosed. Learn more on our breast enlargement page, which explains what non-surgical care can and cannot do.

05Why breasts may not develop as expected

Delayed or limited breast development can have several causes. Some girls simply have a later puberty that runs in the family. Low body weight, intense sports training, eating disorders and chronic illness can delay puberty. Hormonal causes include low oestrogen from ovarian problems, pituitary disorders, thyroid disease and, rarely, genetic conditions such as Turner syndrome. Very marked asymmetry can result from a congenital difference in chest wall muscles. A doctor's evaluation identifies whether a medical cause needs treatment or whether development is within the normal range.

06Breast changes after pregnancy and weight loss

After breastfeeding or significant weight loss, many women notice smaller, softer or sagging breasts. This is due to changes in glandular tissue and skin elasticity, not a disease. Supportive bras, strength training for the chest and back muscles, and maintaining a stable weight help appearance and comfort. Surgical options exist for women who want them; discuss realistic outcomes with a qualified plastic surgeon. Be wary of creams and pills that claim to restore size.

07Gynaecomastia in teenage boys

Up to half of boys develop some breast tissue during puberty, often tender and sometimes on one side. In most, it settles within 6 months to 2 years without treatment. It is helpful to reassure the boy, check for medicines or supplements (including protein powders with additives or steroids), and monitor. Persistent, large or distressing gynaecomastia, or development with other signs of hormonal imbalance, should be evaluated by a doctor.

08Questions to ask your doctor

  • Is my breast development within the normal range for my age?
  • Which hormone tests do I need, if any?
  • Could a medicine I am taking be affecting my breasts?
  • What are the safe options if I am unhappy with size or shape?
  • Which changes should prompt me to come back quickly?

09Breast enlargement in men (gynaecomastia)

Gynaecomastia is an increase in glandular breast tissue in males. It is common in newborns, during puberty (often resolving within two years) and in older men. Causes include hormonal imbalance, obesity, certain medicines, anabolic steroids, alcohol, liver or kidney disease, and thyroid problems. A doctor examines the tissue, reviews medicines and may order hormone tests. Persistent gynaecomastia can be treated with medicines in early stages or surgery.

10Tests that may be ordered

  • Hormone tests: oestrogen, testosterone, LH, FSH, prolactin, thyroid.
  • Liver and kidney function in men with gynaecomastia.
  • Breast ultrasound for lumps; mammography for women over 40 as advised.

11Breast health habits

Know how your breasts normally look and feel, and report changes early. Maintain a healthy weight, exercise, limit alcohol and avoid smoking. Women over 40 should discuss screening mammography with their doctor. Read why breast pain occurs.

13Self-confidence and body image

Concerns about breast size or shape can affect self-esteem, relationships and willingness to exercise or socialise. It may help to remember that breast size varies widely and is not related to femininity, health or the ability to breastfeed. A well-fitted bra, posture work and strength training improve comfort and appearance. If worries are intense, persistent or lead to avoidance, talking to a counsellor can help, and a doctor can explain safe options honestly.
